AirCare Labs LLC had been functioning as a software and technology company developing communciation systems designed to reduce patient readmission to hospitals. Sometime during FY16, AIrCare Labs was acquired by PA firm RightCare Solutions and the SBIR project was moved to the acquiring firm. AirCare had been developing a software platform to faciliates post-discharge communciation between a patient and the health system treating them. Using simple questions easily answered by the patients, health care systems were able quickly to spot potential issues and address them before they require a hospital readmission. The system worked through either daily home software interactions or automated phone calls with the platform's telehealth abilities allowing doctors and nurses to follow-up as necessary. AirCare was also pioneering communication methods, such as video chat, to facilitate patient followup.
