This project develops and tests the feasibility of a system that automates the administration and scoring of the medical outcomes study short-form 36 (sf-36) surveys. The sf-36 measures patientsÂ’ functioning and well being, and thus addresses the potential of individuals with kidney failure to live independently and contribute to the paid workforce. With this approach: (1) patients who have difficulty completing questionnaires because of low vision or low literacy are able to complete the sf-36 by telephone, unaided, and (2) patient data are available to the social workers, nurses, therapists, and physicians on a secure web site.
