This project surveys 50 disabled persons for what information is needed in a Health Care Provider (H.C.P.) data base. Health Care Providers include vendors, agencies and institutions, and rehabilitative centers which service disabled persons. Responses to the survey will help determine the structure and content of the data base. Twenty-five Health Care Providers will be requested to provide information which will be used to make a diskette containing information about each H.C.P.. Programs are written that create screens with which the user can interact. One screen introduces the system to the user. Another allows the user to select H.C.P.'s from the diskette to include in his/her data base. Another allows the user to access H.C.P. information from his/her data base.Anticipated Results and

Potential Commercial Applications:
Phase I should show the basic approach to be sound and should add information for the design of Phase II.Phase II should result in a robust system which will handle virtually all H.C.P.'s in the U.S.A., and all disabilities that these H.C.P s service. Particular emphasis will be placed on making the interface program as usable as possible by people of all disabilities.Commercial applications include the marketing of the system to all disabled people and their families; marketing of this or a similar system to organizations, institutions, rehabilitation centers and vendors; marketing update of the individual diskettes; marketing updates of the system software.Key Words: Disabled, Database, P.A.C.T., H.C.P.Topic 4: Development of Management Information System Software Concerned with the Needs of Disabled Individuals and their Families
